In a city this size, the assistant has to choose 3.
The competition is not the businesses above you. It is the length of the answer.
Why a big city changes the problem
In a small town an assistant naming local businesses may only have a handful to describe. In Milan it can have hundreds for the same question, and it still answers with 3.
So the gap between being known and being named is wider here than anywhere else in Italy, and it does not close by being slightly better.
What gets a business onto a short list
Something has to separate it in a sentence. A neighbourhood rather than the city. A specific service rather than the trade. A kind of customer, a language spoken, an opening hour nobody else keeps.
Assistants describe businesses in one or two lines. A business that cannot be described in one line is hard to put in an answer at all.
Milan asks in more than one language
The city works in Italian and a lot of business in it happens in English. Those are 2 different questions, and assistants answer them from the material they can read in each.
A site that exists only in Italian is absent from half the question, in the one Italian city where that half is large.
The neighbourhood is the real unit
Nobody who lives here searches for the whole city. They search Navigli, Isola, Porta Romana, Lambrate, and they mean 15 minutes on foot.
A page that names the area it serves is answering a question that actually gets asked, and it is competing with a few dozen businesses instead of a few thousand.
Check it before you spend anything
Ask 3 assistants for a recommendation in your trade and your neighbourhood. Write down who they name and the date beside it.
Do it again for the whole city and compare. If you appear in one and not the other, you already know which page to write.
Questions worth asking
We are outside the centre. Does that hurt?
For a city question, yes. For a neighbourhood question it is the reason you would be named at all, and neighbourhood questions are the ones people actually ask.
Is this different from SEO in Milan?
The work overlaps and the outcome does not. Search shows a list you can appear on at position 8; an answer names 3 businesses and does not mention the rest.
Do we need an English version of the site?
In Milan it is worth more than in most Italian cities, because a real share of the buying happens in English. Elsewhere it is usually a later job.
How long does any of this take?
Publishing an answerable page takes an afternoon. Being picked up depends on the assistant, and the ones working from training data are on a schedule nobody outside publishes.
